Character state is harder because it includes more than appearance or injury. It includes what a character knows, believes, has been told, has witnessed, and has forgotten. World rules govern established constraints, whether they involve magic, technology, law, geography, religion, or social hierarchy. For each major character, record what they know at the start of every scene and what they learn during it. Flashbacks create trouble. A character can know an event in the story's present while the reader learns it later, but the narration must preserve the difference between reader knowledge and character knowledge. Test the rules

Run every major scene against the world's established constraints. If magic carries a cost, confirm that the cost is paid. The best check happens at first establishment. Record the fact when it appears, then compare every later scene against that running canon.
